For medically necessary Medicare Part D approved drugs

With MVP Gold  

Once your total drug expenses reach $2,700, you will pay for the full cost of the prescriptions. When you have paid $4,350 out of pocket, your cost for prescriptions is reduced to 5% or $2.40 for Generics and $6.00 for Brand-name drugs, whichever is greater.

With MVP GoldAnywhere (in or out of network)

Once your total drug expenses reach $2,700, you will pay for the full cost of the prescriptions except for generic prescriptions, which will continue to be $5 . When you have paid $4,350 out of pocket, your cost for prescriptions is reduced to 5% or $2.40 for generics and $6.00 for brand-name drugs, whichever is greater.

Low Income Subsidy (LIS) Premium Summary Chart
For people with limited incomes and resources, extra help paying for a Medicare prescription drug plan is available. Information regarding this program is available through the Social Security Administration (SSA). For more information about this extra help, visit SSA at www.social.security.gov or call them at 1-800-772-1213 (TTY 1-800-325-0778).

Extra Help for Paying Part D Costs

The Medicare Modernization Act of 2003 added a prescription drug benefit to Medicare, called Medicare Part D.  The law also provides for financial help with the cost of Medicare Part D prescription drugs for people with low incomes who meet certain income and asset qualifications.  This assistance, called “extra help,” is also known as the Low Income Subsidy (LIS) program.

The LIS program helps pay for some Part D prescription drug plan costs, including Part D premiums, deductibles and copayments.  Depending on your income and assets, you may qualify for the full subsidy or a partial subsidy.  For more information about the LIS program, call 1-800-MEDICARE (1-800-633-4227) or your local Social Security office.
