Become An MVP Broker

MVP Broker Program
Broker Initial Acceptance Process
In order for a broker to become a participating member of the MVP Broker Program, the following
procedures are required:
- Obtain from and submit to the Director of Broker Programs a completed
Broker Questionnaire (PDF).
- Contact the appropriate dedicated MVP Account Executive to obtain a rate quote for the new
prospective group.
- Once the group is sold, the broker should obtain a Broker of Record Letter from the group and
complete the Commission Check Remittal form (PDF). Both of these documents should be submitted to your
dedicated Account Executive who will forward them along with a copy of the MVP Data Entry form to
the Director of Broker Programs. Upon receipt of these three (3) items, the Director will
issue a broker number.
- The Director will then send an MVP Broker Agreement and a full disclosure of Policy and
Administrative Guidelines to the newly appointed MVP broker.
- As an accepted member of MVP's Broker Program, any future group of two or more eligible members
may be submitted by the broker and, if accepted by MVP, commission will be paid on that accepted group.
